Available Levels of Care at Palm Partners
Palm Partners has various levels of care. Our levels of care range from a highly structured environment to an environment less structured and more flexible to meet the needs of the individual. Level of care is assessed at the time of pre-admission and admission.
This is a multi-disciplinary program that is geared towards individuals who are in need of sub-acute medical care and services as a result of their drug or alcohol dependency. Emphasis is placed on the careful medical monitoring of individuals, on either a residential or an outpatient basis, for the purpose of stabilizing the physiological and psychological effects of the withdrawal process. The program offers a comprehensive range of services to assist individuals in understanding the addiction process and the treatment options available to sustain a successful sobriety.
This is a highly structured program that includes a planned regimen of evaluation, observation, medical monitoring, and clinical protocols delivered through an interdisciplinary team approach provided 24 hours per day, 7 days per week in a highly structured, live-in environment. The program is geared toward individuals who have been determined to be at very high risk of relapse due to severe impairments in levels of functioning as a result a co-occurring psychiatric condition or as a result of erratic and unstable symptoms as a result of post-acute withdrawal.
This level of care is appropriate for those in treatment and relapse prone clients. Residential treatment involves the use of solution oriented traditional psychotherapy (individual, family, and group), hypnotherapy, yoga, meditation, exercise, nutritional counseling, recreation, relapse prevention education and therapy, and a 12 step recovery model. Our residential program has been developed to enhance the individual’s potential for on-going recovery. It is both intensive and extensive in the number of client contact hours, the issues covered, and the solutions generated.
This is an intensive structured program where residents remain at the facility in a safe, supportive environment while receiving intensive treatment. This gives one the opportunity to become employed in the community and develop a strong support network. This program is designed for those individuals who are currently stable but are at a high risk for relapse. Emphasis is placed on continued stabilization and intensive supportive treatment.
This program is for individuals who are in need of intensive structured treatment but are currently employed in the local area and/or have a stable place of residence in the community. The intensive outpatient program allows an individual who is currently stable to meet their rehabilitation needs while continuing in their current employment and living situation.
This program is designed for those individuals who have completed formalized treatment and have made a successful transition back into the community. This program provides support geared toward the further development of recovery related program activities both for Palm Partners and the community. Additionally, aftercare serves as a program to continue to refine recovery plans and monitor post treatment.
- Extended Care Program
Extended care simply refers to a setting where a person can reside and receive the necessary support that will ensure a successful recovery lifestyle. This program offers a variety of sober living residences including 1/2 way housing, 3/4 housing, and apartments that will afford an individual both a comfortable, supportive setting and sober supports.
